Metropolitan Veterinary Referral Services

Our team of doctors, nurse technicians and support staff are dedicated to providing the highest quality of service to you, to your pet and to your primary care veterinarian who has confidence in our ability to help you and your pet. Our multidisciplinary staff of internists, emergency doctors, surgeons, physical therapists and highly trained nurses will insure that your pet receives the most comprehensive and compassionate care in the available in the Twin Cities.

We are staffed 24-hours a day, seven days a week. We believe that we can help you.

Patient Information Form

You may download the patient information form, fill it out and bring it with you on the day of your appointment.

FOR YOUR APPOINTMENT:

1)   Bring all significant information about your pet including:

• the referral form filled out by your veterinarian
• x-rays
• blood test results
• Special diet or medications your pet is taking

2)   Please, remember that the information your veterinarian provides to us prior to your appointment will help determine how best to help your pet.

3)   Arrive 10 - 15 minutes early to check in. Bring the completed Patient Information Form with you.

4)   Do Not feed your pet for 12 hours prior to your appointment. Exceptions to this recommendation would be diabetic patients and your puppies or kittens. If you are uncertain, call the office and our friendly reception staff will advise you. You should give all normal medications. Offering water is fine.

5)   If you are unable to keep your appointment, please give us a 24-hour notice. We'll be happy to reschedule.
